Please bring the following items to your appointment:
- photo ID
- insurance card
- a list of your medications
- reports and/or images that may be relevant to your sinus issues

Physician Referrals
Physician referrals are only necessary if your insurance company requires one. Please contact your insurance provider before booking an appointment if you are unsure whether a referral is required.
Accepted Insurance
Our friendly office staff is always available to answer any questions about your procedures and to verify insurance coverage. We accept most all insurance. Though your insurance may not be listed below, we may – in fact – accept it. Please contact us if you have a question about insurance.
